In the Process

There are four components to completing the Health Portal: the Belmont Medical History form; uploading a copy of your immunization record; uploading a copy of your insurance card (front and back); and transcribing immunization dates into the Health Portal. This page will assist you in determining where you are in the process. All operations within this page rely on you being in your Health Portal. Instructions to access the Health Portal are as follows:

  • Log-in to myBelmont with your username and password.
  • Click on the "Personal Records" Section located on the left side of your screen, then "Health Portal."
  • Carefully read instructions and then click on the "Go to Health Portal" button.

Before you begin

  • Please be sure you are using either Firefox or Chrome as the Health Portal does not operate well with Internet Explorer.
  • Check the “Messages” tab for any communication from Health Services.
  • The easiest way to upload required documents is by taking a picture of the necessary document with your Smartphone and uploading the image.

Belmont Medical History

  • In the Health Portal, select “Forms.”
  • If the message “Completed on (date)” appears beside “Belmont Medical History”, then you have completed the Belmont Medical History form.
  • If the message “Completed on (date)” does not appear, you need to complete the Belmont Medical History form. Please be sure you answer every field of this form; otherwise, the system will not accept the form.

Immunization Record

  • Select “Upload.”
  • At the bottom of this screen, under “Documents already on file,” a list of documents already uploaded will appear; you may open any document previously uploaded.
  • Please open and review the documents you have uploaded.
  • If a copy of your immunization record is present, please be sure you have transcribed dates into the Health Portal (see instructions below).
  • If a copy of your immunization record is not present, please upload a copy of your immunization record. Without your immunization record, we cannot verify your immunizations. The easiest way to upload is by taking a picture with your Smartphone and uploading the image. Further, please be sure you have transcribed dates into the Health Portal (see instructions below).
  • If you have uploaded a copy of your immunization record, but have not transcribed the immunizations into the Portal, you still must transcribe your dates. Do this by selecting “Immunization” and “Enter Dates.” Please transcribe all required and relevant immunization dates into the Portal.

Transcribing Immunization Dates

  • Select “Immunizations” and press “Print History.”
  • If immunizations appear on the pdf and the word "Compliant" appears on this document, you have completed the Health Services requirements. Please check your messages for any communication.
  • If immunizations appear on the pdf in this new tab and have a “No” in the “Verified” column, then your immunizations have been entered, but not been verified. Please be sure you have uploaded a copy of your immunization record so that your immunizations may be verified.
  • If immunizations appear on the pdf in this new tab and have a “Yes” in the “Verified” column, your immunizations have been verified. Please check your “Messages” tab for any messages from Health Services.
  • If immunizations appear on the pdf in this new tab and have a “No” in the “Verified” column, and you have uploaded a copy of your Immunization Record, then our staff is still processing your records.
  • If no immunizations appear on the pdf in this new tab, you need to enter dates for each required immunization. Please do so and then upload a copy of your immunization record.

Insurance Card

  • Select “Upload.”
  • At the bottom of this screen, under “Documents already on file,” a list of documents already uploaded will appear; you may open any document previously uploaded.
  • Please review the documents you have uploaded.
  • If copies of the front and back of your insurance card are present, you have satisfied this requirement.
  • If copies of the front and back of your insurance card are not present, please upload copies of the front and back of your medical insurance card. The easiest way to do this is by taking a picture with your Smartphone and uploading the image.
