Immunization Requirements

The information listed below is to assist incoming Occupational Therapy students with completing and submitting the required immunization records to Belmont University and the School of Occupational Therapy. Some School of Occupational Therapy requirements differ from the University’s, many of these are due to requirements of clinical training sites used in the School of Occupational Therapy’s Fieldwork Education.

All OT students must comply with the exact requirement set forth below. Once submitted, School of OT will retain a copy of each student’s record to verify health and immunization compliance and to satisfy any audit requests from site administrators. All students are required to retain a copy of any submitted documentation for their own personal records as well.  These requirements are not only for the health and safety of our students but also for the safety of the patients they will be serving outside of the University.

The immunizations required are following:

  • Either a negative TB skin test within the past 12 months or a chest x-ray if there is a history of positive TB skin test. This must be kept current each year while in our program.
  • Tetanus or Tdap booster within 10 years of the first day of class.
  • Two Measles, Mumps, & Rubella (MMR) vaccinations for those born after 1956, or positive blood tests (titers).
  • Completed series of three (3) Hepatitis-B vaccines, positive blood test (titer), or documentation of informed refusal of the vaccine The series must be completed on schedule. If you refuse the vaccine, please know that you may do so for the program, however your fieldwork placements may be affected as most sites will not accept someone without the completed vaccine.
  • Two doses of Varicella (chicken pox) vaccine, or positive blood test (titer). History of the chicken pox will not satisfy this requirement.
  • Flu Shot (optional) Some fieldwork sites require this, but it is not required for the OT program.



All documents must be submitted to the OT office by July 15th.
