Request Accommodations

Step 1: Have your disability documented by a qualified professional. (See Documentation Guidelines for details).

Step 2: Mail your documentation to:

Step 3: Once your documentation is received you will receive a letter and a Disabilities Accommodations Request Form to complete. Complete, sign, and return the form to the Office of the Dean of Students.

After completing Step 3 your documentation for ADD/ADHD, a learning disability, and/or psychological disability will be verified by a consulting psychiatrist. If your documentation meets the guidelines listed on our website then the university's Disability Program Coordinator will discuss, recommend, and approve accommodations.

For a physical disablity, after completing Step 3 a staff member from Belmont's Health Services will verify your documentation. If your documentation meets the guidelines listed on our website you will meet with the Disability Program Coordinator to discuss possible accommodations needed for your living and learning experience at Belmont.

Step 4: Once you have registered for classes, schedule another meeting with the Disability Program Coordinator to receive your Accommodations Request Form(s) to present to your professors. These forms must be signed by you and the professor and returned to the Office of the Dean of Students. Accommodations are to be requested in a reasonable amount of time prior to the beginning of classes. Step 4 must be repeated each semester.
